	How Do I Become A Resident? Contact one of our Retirement Counselors by calling Westlake Woods at 814 835-0330 or fill out our online information request form to have your name added to our list of prospective residents. A Retirement Counselor will call you to schedule a personal visit to preview our model apartment home. Complete a Resident Application Have your family Physician complete a Westlake Woods Medical Evaluation. One of our Westlake Woods Care Professionals will schedule an appointment with you to conduct a personalized care assessment. Complete a Residency Agreement with Westlake Woods Schedule your “Move-In” day!
