	How Do I Become A Resident? Contact one of our Retirement Counselors by calling SouthWoods at 814 827-8727 or fill out our online information request form to have your name added to our list of prospective residents. A Retirement Counselor will call you to schedule a personal visit to preview our model apartment home. Complete a Resident Application Have your family Physician complete a SouthWoods Medical Evaluation. One of our SouthWoods Care Professionals will schedule an appointment with you to conduct a personalized care assessment. Complete a Residency Agreement with SouthWoods Schedule your “Move-In” day!
