How Do I Become A Resident?

• Contact one of our Retirement Counselors by calling SouthWoods at 814 827-8727 or fill out our online information request form to have your name added to our list of prospective residents.
• A Retirement Counselor will call you to schedule a personal visit to preview our model apartment home.
• Complete a Resident Application
• Have your family Physician complete a SouthWoods Medical Evaluation.
• One of our SouthWoods Care Professionals will schedule an appointment with you to conduct a personalized care assessment.
• Complete a Residency Agreement with SouthWoods
• Schedule your “Move-In” day!
