	How Do I Become A Resident? Contact one of our Retirement Counselors by calling Hawthorne Woods at 724-222-1005 or fill out our online information request form to have your name added to our list of prospective residents. A Retirement Counselor will call you to schedule a personal visit to preview our model apartment home. Complete a Resident Application Have your family Physician complete a Hawthorne Woods Medical Evaluation. One of our Hawthorne Woods Care Professionals will schedule an appointment with you to conduct a personalized care assessment. Complete a Residency Agreement with Hawthorne Woods Schedule your “Move-In” day!
